Privacy Policy & HIPAA Notice
Effective October 15, 2026
PuraVida Health LLC ("we," "us") provides primary care through house calls and telehealth in Florida. This page covers two things: (1) how we handle information collected through puravidahealthmiami.us, and (2) a summary of how we use and protect your protected health information (PHI) as a covered entity under HIPAA. Patients receive our full Notice of Privacy Practices at their first visit, and you can request a copy at any time.
Website information
- Contact form: name, email, phone, visit ZIP code, who the request is for, and your message. Please don't send detailed medical information through the website form or regular email.
- Server logs: IP address, browser type, and pages visited, used for security and troubleshooting.
- Visit prep planner: runs entirely in your browser. We don't receive or store your selections.
- We don't use advertising trackers and don't sell personal information.
How we use and disclose PHI
Once you become a patient, we may use and disclose your PHI without your written authorization for:
- Treatment: providing and coordinating your care, including with specialists, pharmacies, laboratories, and home health agencies.
- Payment: billing your health plan and verifying coverage.
- Health care operations: quality improvement, training, and business management.
- As required or permitted by law: public health reporting, health oversight, legal proceedings, preventing serious threats to health or safety, and other purposes HIPAA allows.
Other uses, including sharing with family members who aren't involved in your care, require your written authorization, which you may revoke in writing at any time.
Your rights
- See and get a copy of your medical record, including electronically.
- Ask us to correct your record.
- Request confidential communications, such as using a different phone number.
- Ask us to limit what we use or share. We aren't required to agree in all cases, but we must agree not to share with your health plan for services you paid for in full out of pocket, if you ask.
- Get a list of certain disclosures we've made.
- Get a paper copy of our Notice of Privacy Practices.
- Choose someone to act for you, such as a health care surrogate or legal guardian.
Telehealth
Video visits use a HIPAA-compliant platform under a business associate agreement. We don't record visits. Please join from a private location.
Security
We use administrative, physical, and technical safeguards such as encrypted records, access controls, and staff training. If a breach of unsecured PHI occurs, we'll notify you as required by law.
Retention
Medical records are kept for at least the period required by Florida law. Website contact form messages are kept only as long as needed to respond and schedule.

If you believe your privacy rights have been violated, contact our privacy officer at support@puravidahealthmiami.us or (305) 947-2186. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We won't retaliate against you for filing a complaint.
