Refund Policy
Effective October 15, 2026
Clinical services are charged when they're provided, so we can't refund care because of a disagreement about the medical outcome. We do correct billing errors promptly and refund overpayments. This policy explains how.
Cancellations and missed visits
- There's no charge if you cancel or reschedule at least 24 hours before a scheduled visit.
- Repeated late cancellations or no-shows for house calls may result in a missed-visit fee, disclosed in writing at intake. Missed-visit fees are never billed to Medicare or Medicaid and are waived for emergencies and hospitalizations.
- If we cancel or our clinician arrives outside your arrival window and you can't be seen, there's no charge.
Insurance overpayments
If you paid a copay or estimate and your plan's final processing shows you owe less, we refund the difference within 30 days of receiving the plan's explanation of benefits. You don't need to ask.
Self-pay services
- Prepaid self-pay visits that are cancelled at least 24 hours in advance are refunded in full.
- If final self-pay charges are substantially higher than your good faith estimate, you may have the right to dispute the bill under federal law. We'll give you information about that process with your estimate.
Billing errors
Tell us within 90 days of a statement. We'll review it, correct confirmed errors with you and your insurer, and refund or credit any overpayment within 10 business days of confirming the error.
How to request a refund or billing review
Email support@puravidahealthmiami.us or call (305) 947-2186 with the patient's name, date of service, and the issue. Please don't email detailed medical information. We acknowledge within 1 business day and respond in writing within 10 business days. Approved refunds go to the original payment method.
Disputes
If you disagree with our decision, see the dispute resolution section of our Terms of Service.
